You have the right to access your medical records, request amendments, request a confidential channel of communication, request restrictions on disclosures, and request an accounting of disclosures. Routine requests processed within 30 days; complex requests within 60.
Membership is month-to-month or annual at member's choice. Cancellation requires 30 days written notice; annual prepay refunds the unused balance. Membership confers no right to any specific physician's panel; members may transfer between physicians with mutual consent. Full terms provided at the first visit.
